Calling JavaScript from ContentEditor Web Part in Custom List Form

I've created a custom form (TestNewForm.aspx) associated with a list (let's call it Announcements).  I want to be able to use JavaScript to create the interface on this form.  

There is another form (associated with another list) developed by a contractor who is no longer here.  At the bottom of the custom form he has a Content Editor Web Part that calls the JavaScript via a CustomLink tag within the CEWP tag.

When I added the CEWP tag I don't get a CustomLink tag (nor most of the tags that he has).  I'm sure I could copy and paste but I'm guessing that he added the CEWP via Designer which caused most of the tags to appear.

Anyway, I'm looking for information on how to create a custom list form in Designer 2010 and set a link to an external JavaScript file that will format the form.  

Any help is appreciated.
