How to display a list box via lookup

Hi,

I have a Inventory list with some fields like UID (computer name), Model, Software etc. Several stored information as

UID           Model                                     Software                                       ShownOnInventoryServiceLIST

FGS_2K3   Dell PowerEdge 2100 Server                                                         Y

FGS_2K3                                                  Microsoft Windows 2003 Server     N

FGS_2K3                                                  Microsoft EXCEL 2010                   N

I would like to setup another list (service report to log down any service call of each inventory) and would like to have a lookup listbox (which can show FGS_2K3 : Dell PowerEdge 2100 Server) maybe use the Y value to determine it will shown on the UID lookup list...

can you please help how to do it ...
